ACAP Report: Safety Net Health Plan Approaches to Addressing Chronic Conditions

A new report from ACAP highlights the innovative measures its Safety Net Health Plans are taking to address their patients’ chronic conditions, while also outlining a suite of policy recommendations to facilitate more effective management of chronic conditions among health plans.

On page 5, ACAP’s new report highlights Community First Health Plans’ partnership with Podimetrics, beginning with a commercial pilot and expanding to adult Medicaid members. Eligible members are invited to participate and supported through a simple daily monitoring routine designed to help identify risk earlier, before complications escalate.

Early qualitative feedback noted the program’s convenience and ease of use, and expected outcomes include improved member satisfaction, reduced inpatient hospitalizations, and decreased emergency department utilization.
